Surgical treatments for obesity, such as Roux-en-Y gastric bypass and sleeve gastrectomy, aim to promote significant weight loss by reducing stomach size and altering digestion. These procedures are typically performed using minimally invasive techniques, either through keyhole (laparoscopic) surgery or advanced robotic surgery. Both methods involve small incisions, leading to quicker recovery, less postoperative pain, and reduced risk of complications compared to traditional open surgery.
Roux-en-Y Gastric Bypass
The Roux-en-Y gastric bypass, commonly referred to as gastric bypass surgery, involves two key steps. First, a small stomach pouch, also known as a gastric reservoir, is created using a stapler. This pouch has a capacity of approximately 10 to 30 ml, while the remainder of the stomach is left intact but no longer participates in digestion.
In the second step, the small intestine is divided about 30–45 cm from its origin, and the lower segment is connected to the gastric pouch. A further 120 cm down, this segment of the small intestine (the alimentary loop) is joined to the remaining portion of the small intestine (the biliary loop) in a configuration resembling the shape of a "Y," giving the procedure its name.
This reconnection point, located 120 cm beyond the gastric pouch connection, is where food mixes with digestive juices, allowing for partial digestion and absorption. The term "bypass" refers to the exclusion of most of the stomach and the duodenum from the digestive process.

Sleeve Gastrectomy
During a sleeve gastrectomy, approximately 75% of the stomach is surgically removed, leaving a smaller, tube-shaped stomach that connects the oesophagus to the duodenum. This significant reduction in stomach size allows patients to feel full more quickly, leading to reduced food intake.
Additionally, the portion of the stomach that is removed contains the majority of cells responsible for producing ghrelin, a hormone known to stimulate appetite. As a result, ghrelin levels decrease following the procedure, which helps to further reduce appetite and support long-term weight loss.
